A condition in which the upper eyelid becomes droopy and covers the eye is called ptosis. When this condition occurs, the above symptoms appear.

There are two main types of causes of droopy eyelids.
The first is due to sagging skin. This is due to the gradual stretching and sagging of the skin with age.
It is more likely to occur in people who have always had a single eyelid, and deep wrinkles appear on the forehead and the shape of the eyes becomes a figure of eight, giving the face an illustration-like appearance. This is called a drooping eyelid.

On the other hand, there is also a type of droopy eyelid in which the muscles that raise the eyelid become weak. With age, the muscles become elongated, and the eyebrows rise to compensate.
People who wear or have worn hard contact lenses in the past are more prone to this type of drooping eyelid.

The treatment is surgery for each cause.
The Case of Eyelid Drooping caused by sagging skin
In the case of Eyelid Drooping caused by sagging skin, the drooping skin is removed above the lashes to create a double eyelid that will not come down any further. In some cases, fat removal is also performed. If the patient has always had a double eyelid, a submental skin incision may be made just below the eyebrow. In severe cases, both may be necessary.
The Case of Eyelid Drooping caused by muscle weakness
In the case of Eyelid Drooping caused by muscle weakness, the stretched muscles are treated by sewing them back into position so that the eyelids are functionally elevated. The skin may be removed at the same time. In mild cases, this may be done from the back of the eye without incising the skin.

In Eyelid Drooping caused by sagging skin, the goal is to remove excess skin and to create a double eyelid.
An incision is made in the eyelid skin about 5 to 6 mm above the eyelashes, and the excess skin is removed. The double eyelid is then created and the skin is sutured. Fat may be removed at the same time.
The advantage of this method is that the skin is removed and the double eyelid can be remade. If the patient has always had a unilidery or a double folded eyelid, it will be more beautiful if the eyelid is remolded.
Another method of skin excision is to make an incision under the eyebrow and remove the skin (subhyperbrow dermabrasion). If you have thick eyelid skin and anticipate that an incision above the eyelashes will result in a thick and unnatural double eyelid, a subbrow dermabrasion is preferable.
This method exposes your own double eyelids, giving you the natural look of your own eyes about 10 years ago when the skin was sagging. Because the flaws in a subbrow dermabrasion are on the surface of the body, basic competence in the basics of incision and suture technique, in addition to design, is of paramount importance. Without skill, the flaws will be conspicuous. At our clinic, we take the utmost care to make the flaws inconspicuous.

Proper design and stitching requires a high level of skill and extensive experience.
Although it is a simple cut and sew technique, there are tricks to the design and sewing method. Because sagging gradually increases with age, the older one gets, the more the skin around the eyes must be removed to the extent that it exceeds the width of the eye, or the wrinkles around the eyes may appear to have increased after the surgery, conversely.

Eyelid Drooping caused by muscle weakness
Surgery for tendinous ptosis can be improved by sewing the stretched tendon back into its youthful position (levator muscle resection).
Surgery may also include removal of excess skin and fat that is causing heaviness.
The surgery takes about 10 to 30 minutes per side depending on the severity of the surgery, but we use intravenous anesthesia sedation to ensure that the patient does not feel any pain or time.

After surgery

Side effects, risks, and precautions for ptosis treatment
The surgery is mainly performed under local anesthesia to check the shape of the eyelids during the surgery, but general anesthesia may be used depending on the situation, such as in children.
Sedatives may be used to reduce pain before surgery.
When surgery is performed under local anesthesia, the patient may experience severe pain. If the pain is painful, additional anesthetics or painkillers may be administered.
Patients who have undergone multiple surgeries tend to have more pain.
Although the surgery is performed to minimize the left-right difference as much as possible, it is not possible to achieve perfect symmetry. If there is a large difference between the left and right sides after the surgery, we may perform suture treatment again or reoperate.
The scars will gradually become less noticeable after the surgery, but scars may still be visible and keloids may occur.
If the wound is dissected after surgery, it must be sutured again.
Orbital cellulitis may occur due to infection.
Intraoperative sedation may cause hypotension, pulselessness, and respiratory depression.
